How do you reference your evidence for you awarded scale points? Do you have a cover sheet saying what you have and where it is?

I have a sheet with all the profile points in the back of their learning stories, every observation I do I ref e.g. CLL (LSL) 3. I then tick it off at the back of the learning story. I also write if it is Child Initiated or Adult Led and only highlight the sclae point when I have at least pieces of evidence at least 2 of which are CI. xx
