I am currently childminding a 7 year old with ADHD and developmental delay.

She is with me for 2 or 3 hours a day, while decisions are made about moving from mainstream to special school.

She is on once-a-day medication (20mg) which Mum gives her each morning BEFORE she goes to school and/or me.

I have now been asked to make sure I have a Care Plan in place, in case of emergency.

The sample plans I have are all medical based and nothing to do with emotional or behavioural issues. I know there should be one in place at school but as there are "issues" regarding school I don't feel I can approach them to ask for a copy huh.gif

Does anyone have a Care Plan they use that I could have a look at PLEASE?

I expect you could ask for a copy from the school, given that we are supposed to be swapping useful information with other settings. My care plans are virtually blank anyway, I write it in conjunction with the parents on an in case this happens ..... basis and they tell me how the best way to care for their child is and how they would react in a given situation.

I've got all the information on other Registration Forms but have been asked to draw up a specific Care Plan form to be kept with me at all times, when out and about with the child so that in the event of an emergency everyone would know what medication she is taking.

However, ideally they'd like me to get Mum to sign one EVERY day to say that the medication has been given (including doseage and time)

I don't always see Mum. Sometimes the grandparents bring and collect the child and other days I pick her up from her adult sister. I wonder if a letter wriiten, and signed, by Mum saying what and when she gives her would suffice? I could then keep this with me when she is in my care.

An approach to school may be tricky as there are ongoing investigations into their handling of this childs needs.

For one of my little chaps a couple of years ago who was on medication every day, I gave mum some photocopies of all the relevant dosage info and she just signed a fresh one every day is that possible?

The one I have is for Nut Allergy but you can adapt it!
